Family Resource Coordinator

Actual pay rate will be calculated based on relevant years of experience and education/certification requirements.
ROLE SUMMARYProvides and maintains information about available services from Children's Village, partner agencies, and the community for children and family members.
Provides information regarding the use of the health care system for children.
Provides assistance to patients and families to help them secure primary care and referrals to other supportive services, access entitlements and insurance, and complete forms and applications.
ESSENTIAL RESPONSIBILITIES Gathers information from all sources to confirm the family's needs are being appropriately addressed, including review of medical records, communication with the team of providers, and input from the family.
Provides support to families through advocacy by assisting in identifying concerns priorities and resources, anticipating informational needs, assisting with completion of appropriate application forms, and assessing comprehension and engagement.
Provides information to families about resource availability, costs, and coverage for care/services in a timely manner.
Describes programs clearly to the parents of eligible children by participating in developmental evaluations to determine eligibility in appropriate programs, developing a plan with the family to coordinate community resource to meet the family's needs, and coordinating referrals to local school districts.
Monitors individual plans on an ongoing basis including reviewing progress toward outcomes, objectively and accurately identifying potential/real barriers, linking families to appropriate community resources, amending service plans as needed to reflect changes in frequency, setting, or duration.
Facilitates collaborative team meetings and case conferences as well as works with multi-disciplinary team to provide family-centered services at Children's Village.
Participates in education opportunities to expand and improve job knowledge and skills as well as maintains knowledge of mandated reporting procedures to Child Protective Services and reviews any incidents with direct supervisor.
Responds to changes in the work schedule as necessary; demonstrates flexibility in view of interruptions; functions as a steadily productive employee.
Plays an active role in encouraging and communicating community development internally and externally.
Travels to homes/community to support child's natural environment for services and supports.
QUALIFICATIONS AND REQUIREMENTSEducation:
Bachelor's degree in nursing, social work, education, or related field required.
Experience:
2 years related experience preferred.
Licenses/Certificates/Registration:
Washington State registration as a Family Resources Coordinator required within 6 months of hire.
Valid driver's license and proof of automobile insurance required.
Level of Business Knowledge/
Knowledge/Skills/Abilities:
Knowledge of Yakima County community resources preferred.
Bilingual English/Spanish language skills required.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
