Social Worker (Bachelors Degree)

Actual pay rate will be calculated based on relevant years of experience and education/certification requirements.
ROLE SUMMARYIn collaboration with the patient, family, physicians, interdisciplinary team and external agencies, the Social Worker ensures the patients progress along the continuum in an efficient and costeffective manner.
The Social Worker is responsible for identifying and initiating the most efficient discharge plan to expedite the patient transition to the next appropriate level of care and to reduce length of stay.
This position is the organizations expert in assessing and managing the patients psycho-social needs.
Utilizes strong clinical expertise, extensive problem-solving capability, and independent decision-making skills.
The Social Worker participates in staff education/development and quality improvement projects as required by regulatory and accreditation agencies.
Maintains significant knowledge and understanding of community and post-acute resources, and related payer requirements.
Educates the health care team and physicians about psycho-social issues and identified patient/family problems, as well as recommends strategies to address these issues.

ESSENTIAL RESPONSIBILITIES Coordinates with Utilization Management nurses and/or Case Managers for post discharge arrangements and will advocate for the patient in obtaining and negotiating resources to meet patient needs, as necessary.
Interviews patients/families to complete a thorough and timely psycho-social assessment and to evaluate current support systems available to the patient and family.
Identify patient goals.
Completes assessments, per department standards, based upon information from multiple sources including the patient, family, nursing staff, physician, as well as, information discussed in multi-disciplinary rounds.
Documented assessments demonstrate an understanding of the patients functional status and psycho-social needs (if BSW, this function must be approved by a MSW).
Collaborates with the Case Manager to address barriers that may impact or delay the patients discharge plan (if BSW, this must be supervised by a MSW).
Prepares for and participates in multi-disciplinary rounds on assigned nursing unit for the purpose of updating the team on the discharge plan/needs and to identify potential social work referrals (if BSW, must have CM or MSW input).
Appropriately prioritizes cases according to assessed needs.
Performs discharge planning / care coordination to include (but not limited to):
Acute Rehabilitation Long Term Acute Care New Nursing Home or Skilled Nursing Placement Psychiatric or Substance Abuse Placements, Cases involving active psychiatric conditions New dialysis Acute Care Transfers Abuse/neglect Cases Hospice Referrals and Placements Legal issues:
adoptions and guardianship cases in collaboration with legal counsel Specialty program participation Assistance with Advance Directives, as necessary Community Resource needs Financial issues/funding for post-acute needs/ identifying possible MA eligibility cases with appropriate referrals Provides patient/family education.
Adapts quickly to changes in patient status and will update the assessment and discharge plan as the patients situation warrants.
Stays abreast of changing social work clinical trends, regulatory matters and third party payer requirements related to discharge planning and precertification of aftercare benefits.
If MSW or LICSW, provides adjustment to illness counseling, grief counseling and crisis intervention.
(MSW and LICSW)QUALIFICATIONS AND REQUIREMENTSEducation:
Bachelors in Social Services, Social Work, Psychology, Sociology, or related field required.
Masters degree in Social Work preferred.
Licensed Certified Social Work (LCSW) preferred.
Experience:
Minimum of one years experience in acute care, discharge planning experience with knowledge of community resources preferred.
Licenses/Certificates/Registration:
Case Management certification (ACM, CCM) preferred.
Washington State (DOH) LICMSW or Washington State (DOH) Agency Affiliated Counselor required within one year of hire.
American Heart Association BLS required for outpatient environments:
Compass Care, Early Intervention, Transitions, Cleft Lip & Palate, North Star Lodge/Oncology Care, Signal Health & Spiritual Care) Valid drivers license and proof of automobile insurance required for outpatient environments.
Cell phone for use during working hours, car for travel to and from home visits during shift required for outpatient environments.
